Merlin Day Academy Enrollment

Who We Are: Welcome to Merlin Day Academy! We are a multi-disciplinary program for neurodiverse learners aged 6 to 14 providing an intensive, individualized educational and therapeutic day. Our learning environment cultivates relationship-based, family-centered care to strengthen educational and functional skills for students to become active, successful participants in their community.   

What Our Program Provides: Merlin Day Academy collaborates with Eyas Landing to provide an intensive multidisciplinary approach to therapeutic services and academic instruction. This approach means children have daily access and support from the same team of therapists in a natural, peer-oriented environment. The therapeutic rotations are blended with education instruction to provide a strong foundation for supporting the individual with intensive bursts of intervention, interpersonal peer social interactions, and a more consistent schedule. We believe our blend of intensive therapy and education instruction best supports our children’s growth in the most supportive, consistent, and naturalistic learning environment.  

Enrollment Next Steps

Step 1: Complete the registration form and speak to our Office Manager.

If you were previously a Blue Bird Day client, complete the authorization to exchange Protected Health Information form to allow the Blue Bird team to communicate their recommendation to Merlin Day Academy. The BBD Therapy Case Manager will then complete the Merlin Day Academy registration form on your behalf.

Step 2: Access your personal client portal to upload a copy of your insurance card in order to receive your personal benefit verification (See our Guide to Healthcare Coverage for more information on insurance). 

Step 3: Schedule and complete a tour with our Merlin Lead or Merlin Director, where you’ll review our tour packet and financial information. 

Step 4: Receive an estimated cost breakdown of patient responsibility for the program and payment options from your Administrative Case Manager. 

Step 5: Schedule and complete a Meet and Greet with you, your child, and a Therapy Case Manager. 

Step 6: Save your spot with deposit and registration and receive a tentative start date from our Office Manager. 

Step 7: Complete the enrollment packet and credit card authorization (assistance is available from your Administrative Case Manager, if needed). 

Step 8: Upload all past evaluations and medical documents, ie. Asthma/allergy action plans, medication consent to your CentralReach portal (assistance is available from your Administrative Case Manager, if needed). 

Step 9: Schedule and complete Multi-Disciplinary Team Evaluation scheduled by your Administrative Case Manager. 

Step 10: Receive individualized schedule and confirmed start date from your Administrative Case Manager and Office Manager. 

Step 11: Start Merlin Day Academy!

Step 12: Schedule and complete Individualized Intensive Therapy Service Plan (IITSP) Meeting with team, scheduled by your Administrative Case Manager. 

CentralReach Guides

We use the CentralReach platform for client medical records, file management, scheduling, billing and invoicing. Caregivers can look up treatment notes, monitor their child’s treatment, check on invoices and pay for services.

Daily Connect Guides

We use the online application Daily Connect (app) to communicate about your child’s day. Caregivers are able to see their child’s daily activities, receive and send messages to the team, exchange photos, and more. In addition, at the end of the day a summary will also be emailed to all caregivers on the account.
